Completed HIPAA form – Click Here to Download Form (BlueCross Clients Only) Over the past year, you’ve probably heard a lot about legislation designed to protect an individual’s personal health information. Known as the Health Insurance Portability and Accountability Act (HIPAA), this legislation provides standards for how Protected Health Information, or PHI, is collected, used and shared.
3. Explanation of Benefits, if received from your medical carrier.
4. Copy of Bill from provider with Date of Service
5. Patient’s Name, Date of Birth & Social Security Number
